As a skilled Registered Nurse, you will play a vital role in managing patient care and providing services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education. In addition to excellent written and verbal communication, problem-solving, and decision-making abilities, the ideal candidate should demonstrate experience in developing and maintaining rapport with patients and families, as well as the ability to work effectively with an interdisciplinary team.
Key Responsibilities:- Manage patient care and provide services that require specialized bedside nursing skills.
- Develop and maintain rapport with patients and families.
- Work effectively with an interdisciplinary team.
- Provide education and support to patients and families.
The Case Manager RN will primarily work in the patient's residence, with occasional travel to long-term care facilities, hospitals, and area agencies and businesses. Overnight travel outside of the service area may be necessary for continuing education.
