Palliative Care Nurse Practitioner Job Summary

We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our team at Compassus. As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals with serious illnesses.

Key Responsibilities:
  • Provide comprehensive assessments, disease education, and symptom management to patients and their families.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and professional development to stay current with best practices in palliative care.
  • Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Master's degree in Nursing required.
  • Minimum of five (5) years nursing experience, with clinical experience in oncology, hospice, geriatrics, or intensive care preferred.
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Board Certification in Hospice and Palliative Care is preferred, but not required.
