Palliative Care Nurse Practitioner Job Summary

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at FC Compassus LLC. As a Palliative Care Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals with serious illnesses.

Key Responsibilities:
  • Assess patients' physical, emotional, and spiritual needs and develop individualized care plans.
  • Provide symptom management and pain control, as well as education and support to patients and their families.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and chaplains, to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Master's degree in Nursing required.
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Board Certification in Hospice and Palliative Care is preferred, but not required.
Preferred Qualifications:
  • Minimum of five (5) years nursing experience required; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
  • Strong organizational and time management skills.
  • Ability to maintain confidentiality of information, such as patient and Company files.
Work Environment:

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.

Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.

Routine use of standard medical equipment and tools associated with clinical care is essential.
