Care Coordinator

SR COMPANIES LLC is seeking a highly skilled Care Coordinator to enhance and improve the SRIM experience for our residents and their families. This role is responsible for demonstrating and promoting SRIM's core value of the Golden Rule, accepting management responsibilities of the Nursing Department in the absence of the director of resident services, and assisting in managing the care of all residents.
Key Responsibilities- Develop and implement effective care plans to meet the unique needs of each resident.
- Collaborate with the nursing team to ensure seamless care delivery and address any concerns or issues that may arise.
- Conduct regular meetings with staff to discuss resident care, address concerns, and provide feedback.
- Assist in the recruitment and training of new staff members to ensure they are equipped to provide high-quality care.
- Monitor and manage the department's budget to ensure compliance with established guidelines.
- Develop and implement policies and procedures to ensure the highest level of care and safety for residents.
- Participate in monthly ECC & LNS meetings with the Director of Resident Services and Corporate Director of Nursing to discuss resident care and departmental performance.
- Ensure accurate and timely documentation of resident care and services.
- Assist in the investigation of incidents and recommend appropriate action to the Executive Director.
- High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
- Valid Florida Nursing License or Medical Technician Certification required.
- One to two years of experience in a related field, preferably in a supervisory or management role.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Strong problem-solving and analytical skills, with the ability to think critically and make sound decisions.
The work environment is fast-paced and dynamic, with a focus on providing high-quality care to residents. The Care Coordinator will be required to work in a variety of settings, including the nursing department, resident rooms, and other areas of the facility. The position requires the ability to lift and move up to 50 pounds, as well as stand, walk, and use hands and arms for extended periods.
