Job Summary

We are looking for a dedicated and detail-oriented Registered Nurse with experience in coordinating care for patients with neurological conditions. As a Neurology RN Care Coordinator, you will play a vital role in ensuring the smooth delivery of patient care, from initial assessment to discharge planning.

About the Role

• You will be responsible for managing patient flow, triaging cases, and providing direct patient care in a fast-paced clinical setting.
• Your exceptional communication skills will enable you to liaise effectively between front desk staff, nursing teams, physicians, and clinic managers.
• You will work closely with healthcare providers to develop personalized patient plans, define resource utilization, and set achievable goals.
• In this critical role, you will provide comprehensive nursing support and coordination to optimize patient outcomes and enhance their quality of life through clinical programs, educational initiatives, and support services.

Responsibilities and Essential Job Functions

• Manage patient flow, triage, and direct patient care in a high-pressure environment.
• Liaise between front desk staff, nursing teams, physicians, and clinic managers to ensure seamless care delivery.
• Collaborate with healthcare providers to develop individualized patient plans, allocate resources, and establish realistic objectives.
• Offer thorough nursing support and coordination to enhance patient well-being through evidence-based practices, patient education, and support services.

Requirements and Qualifications

• A Bachelor's degree in Nursing is required, along with a minimum of 2 years of relevant RN experience.
• Prior medical office experience is a significant asset.

Preferred Qualifications

• Basic Life Support certification (BLS) from the American Heart Association (AHA).
• Current active multi-state Registered Nurse (RN) license in good standing.

Compensation and Benefits

We offer an attractive compensation package, including a competitive salary range of $80,000 - $110,000 per year, depending on experience. Additionally, our comprehensive benefits include health insurance, retirement savings, paid time off, and professional development opportunities.
