- Our ideal candidate will have a strong background in pediatric and adult care, with experience working with patients with complex medical needs.
Responsibilities:
- Assess patient needs and develop individualized care plans to meet their unique needs and goals.
- Provide high-quality nursing care to patients in their own homes, including medication management, wound care, and vital sign monitoring.
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and communicate with healthcare teams to ensure continuity of care.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
- Current Registered Nurse (RN) License for the state of North Carolina.
- Current Health Certificate.
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior Registered Nurse (RN) experience preferred.
