Job Summary

Indiana Fitness Works, Inc. is seeking a skilled Speech Language Pathologist to join our team in a long term care setting at Persimmon Ridge in Portland, Indiana.

The successful candidate will be responsible for evaluating and treating communication disorders, swallowing disorders, and cognitive deficits in patients, as prescribed by the physician.

Key Responsibilities:

  • Conduct comprehensive evaluations to identify functional speech defects and develop personalized plans of care.
  • Implement and revise plans of care as necessary to improve patient outcomes.
  • Provide education and instruction to patients, family members, and caregivers on recommendations for patient care.
  • Prepare and submit detailed notes per agency policy.
  • Perform other duties as required to facilitate the delivery of speech language pathology services.

Requirements:

  • PRN or part-time position available.
  • Full-time employees may be eligible for benefits, subject to a waiting period.
  • Must be fully vaccinated for Covid-19 or have a medical or religious exemption.
