Home Health RN Full Time Position

Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities:
- Conduct thorough assessments of patients' medical needs and develop personalized care plans in collaboration with physicians and other healthcare professionals.
- Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes, as needed.
- Educate patients and their families on proper care techniques and provide ongoing support to ensure optimal health outcomes.
- Collaborate with primary caregivers to ensure seamless care transitions and provide training as needed.
- Document patient progress and communicate effectively with the healthcare team to ensure timely and accurate care.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
Preferred Qualifications:
- Medicare Skilled Nursing experience.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
