Hospice Registered Nurse Case Manager

At Signature Healthcare at Home, we are seeking a highly skilled and compassionate Hospice Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes and communities.
Key Responsibilities:
- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by the physician.
- Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless transitions between care settings.
- Communicate effectively with patients, families, and healthcare providers to ensure that patient needs are met and that care is coordinated.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality patient care.
Requirements:
- Current licensure as a Registered Nurse in the state of practice.
- Graduate of an accredited school of nursing.
- Minimum of two years' experience in hospice or palliative care nursing, with a focus on case management.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work independently and as part of a team, with a focus on providing high-quality patient care and support.
