Compassionate Hospice Care Specialist

We are seeking a dedicated Registered Nurse to join our team as a Home Healthcare RN Case Manager in Martinsville. This role offers a unique opportunity to provide exceptional care to patients and their families, making a lasting impact on their lives.
About the Role:
- As a Home Healthcare RN Case Manager, you will be responsible for providing vital care to patients and their families, developing personalized plans of care, and ensuring they receive the support they need.
- You will work closely with a passionate team of healthcare professionals, including physicians, social workers, chaplains, and CNAs, to create a cohesive and supportive care environment.
- Your expertise will empower patients, families, and caregivers by educating them on disease management, symptom control, and end-of-life care options.
Responsibilities:
- Perform nursing evaluation visits and craft detailed care plans in collaboration with the physician and interdisciplinary team.
- Continuously assess and re-evaluate patient needs, updating the Plan of Care as needed to ensure the highest quality of life.
- Maintain accurate, up-to-date documentation using our Electronic Medical Record (EMR) system and prepare timely clinical progress notes.
- Participate in interdisciplinary team meetings, contributing valuable insights to enhance patient care.
- Stay ahead in hospice care by embracing best practices, research, and ongoing professional development.
About the Opportunity:
- This is a full-time position with a balanced schedule, offering 4 ten-hour shifts per week and every 3rd weekend rotation, or 8 hour shifts available.

Requirements:
- Valid Registered Nurse License
- BLS Certification
- Valid Driver's License
