About the Role

We are seeking a highly skilled LPN Assistant Clinical Coordinator to join our team at Haven Hospice. As a key member of our clinical operations team, you will play a vital role in supporting the Clinical Supervisor in scheduling staff and managing office functions.

Key Responsibilities
  • Schedule CNA visits for the week in advance using our electronic health record system.
  • Verify and send physician orders for accuracy and completion.
  • Participate in the morning Access call in the absence of the Clinical Supervisor.
  • Assist with physician verbal orders, documentations, and triage RN/SW calls as necessary.
  • Review and track lab results, fax to MD, and place copies in the RN box.
  • Email information for referrals to Access as needed.
  • Email patient information and patient change information to Nurse Triage.
  • Administer PPD or other vaccinations for staff as required.
  • Manage orders for equipment as needed.
  • Order IV meds/supplies from contracted vendors as required.
  • Assist with clinical issues when the Clinical Supervisor is out of the office.
  • Compile communication through telephone calls and contacts as requested by the Clinical Supervisor.
  • Assist with triaging nursing-related calls and dispatching staff.
  • Provide physicians, patients, family members, team members, and other callers with needed information.
  • Inform the Clinical Supervisor of unusual or potentially problematic patient/family issues.
Requirements
  • High School Diploma or GED equivalent required.
  • Advanced education for clinical licensure; successful completion of IV therapy course (30 hours).
  • Minimum of 2-3 years nursing experience required, preferably in oncology, palliative care, geriatrics, or home health care.
  • Experience working with an interdisciplinary team preferred.
  • Requires a current Cardiopulmonary Resuscitation (CPR) certification (American Heart Association).
  • Current driver's license and insurance.
  • Current, unrestricted LPN license in the state of Florida.
  • Position requires expertise in various technologies such as Microsoft Office, electronic medical records, etc.
  • Demonstrated ability to work non-judgmentally with persons of varied cultural, religious, and socioeconomic backgrounds and lifestyles.
