Brookdale Crown Point is seeking a LPN Health and Wellness Coordinator to lead the clinical functions at our community. This role will be a member of the leadership team, working full-time and scheduled on a mid-shift (11am to 7pm) or evening shift (3pm to 11pm) depending on availability.

Responsibilities:

  • Manage day-to-day healthcare operations to ensure residents' healthcare needs are met.

  • Assist in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and caregivers.
  • Manage associates' schedules and oversee continuity and consistency of medication training, pharmacy management, and medication supervision and/or administration.
  • Share on-call duties as required.
