Physician - Internal Medicine Specialist

Under the guidance of the Medical Director or their appointed representative, the Internal Medicine Physician will engage in professional medical practice as an integral member of the Patient Centered Medical Home (PCMH) Team.
The Staff Physician will be responsible for:
- Establishing a continuous relationship with each patient, and where appropriate, their families or caregivers, by ensuring access, continuity, education, and thorough care through accurate diagnosis and treatment.
- Actively promoting organizational goals related to patient care, quality, education, and sustainability.
- Delivering patient-focused services and fostering a positive workplace atmosphere.
- Addressing patients' healthcare requirements and coordinating care with other qualified healthcare professionals as necessary.
- Ensuring quality and safety by adhering to PCMH principles, which include teamwork, evidence-based practices, quality monitoring, and engaging patients in decision-making.
- Providing timely follow-up on patient consultations, including chart documentation, laboratory reviews, and referrals to specialists, along with after-hours coverage as required.
- Participating in team meetings, quality improvement meetings, and other professional activities as directed by the Medical Director.
- Regularly meeting with the Medical Director to discuss patient care matters and contribute to the formulation of treatment guidelines and protocols.
- Offering on-site clinical supervision and support for Family Nurse Practitioners and Physician Assistants.
- Providing support for Family Physicians as necessary.
