**Job Summary**
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Long Term Care setting. As an LPN, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
**Key Responsibilities**
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
- Collaborate with the healthcare team to develop and implement individualized patient care plans
- Participate in quality improvement initiatives to enhance patient care and safety
**Requirements**
- 1 year of experience in a Long Term Care setting
- Current LPN licensure in the state of practice
- Basic Life Support (BLS) certification
