Medical Social Services Specialist

Job Summary:
We are seeking a highly skilled Medical Social Services Specialist to join our team at HexaQuEST Global - Healthcare. As a key member of our healthcare team, you will provide holistic care to patients and their families, addressing physical, social, emotional, and environmental challenges that impact their health.
Key Responsibilities:
- Provide medical social services to patients and their families, utilizing a holistic approach to address their unique needs.
- Collaborate with healthcare professionals to develop and implement patient-centered care plans.
- Conduct assessments, identify needs, and provide interventions to promote optimal health outcomes.
- Develop and maintain relationships with community resources to ensure seamless care transitions.
- Participate in interdisciplinary teams to share knowledge, best practices, and patient outcomes.
Requirements:
- Master of Social Work (MSW) degree from an accredited institution.
- At least one year of experience in a hospital or healthcare setting.
- State of Alaska license at the Master's level (LMSW) or eligibility.
- Valid Alaska State driver's license.
- Basic computer skills and ability to learn new software.
Preferred Qualifications:
- Experience working in cross-cultural settings with bilingual language skills in English and Yup'ik.
- Ability to read, comprehend, and write at the college level, as well as present information effectively in one-on-one and group settings.
Working Conditions:
This is a full-time position with a normal shift schedule. The ideal candidate will be able to work independently and collaboratively as part of a team.
