About the Role

We are seeking a skilled Physical Therapist to join our team at Phoenix Home Care and Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Provide physical therapy services to patients in their homes, in accordance with physician plans of care and under the supervision of a licensed Physical Therapist.
  • Participate in communication and case conference activities to ensure maximum continuity and quality of patient care services.
  • Exhibit sound judgment and technical skills necessary to perform assigned treatments and procedures according to established agency protocols.
  • Maintain compliance with agency standards, policies, procedures, state and federal regulations, and payer requirements.
  • Practice safety, environmental, and infection control measures.
  • Perform miscellaneous job-related duties as assigned by Supervisor.
Requirements
  • Active license in the state of Missouri required.
  • Home health experience preferred, but not required.
  • Competitive pay.
  • Multi-disciplinary collaboration for patient care.
