Job Summary

Aveanna Healthcare seeks a skilled Licensed Practical Nurse (LPN/LVN) to provide high-quality nursing care to patients in a home health setting. The successful candidate will be responsible for assessing patient needs, developing and implementing care plans, and coordinating with healthcare teams to ensure optimal patient outcomes.

Key Responsibilities

  • Deliver skilled nursing care to patients in a home health setting, including trach/vent and overnight support.
  • Assess patient needs and develop individualized care plans in collaboration with healthcare teams.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Document patient progress and care plans in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and coordinate with healthcare providers.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.

Preferred Qualifications

  • 6 months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
