About the Role
We are seeking a highly skilled Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients.
As a RN Case Manager, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote optimal health outcomes and improve the quality of life for our patients.
You will be responsible for performing visits, completing coordination of client care, and serving as the point of contact for all disciplines involved in providing care to patients. You will also consult with physicians and office staff as needed to ensure timely and effective care.
We are committed to providing our patients with the highest level of care and service, and we are seeking a dedicated and compassionate RN to join our team.
Responsibilities
Key responsibilities of this role include:
- Performing visits and completing coordination of client care
- Serving as the point of contact for all disciplines involved in providing care to patients
- Consulting with physicians and office staff as needed
- Developing and implementing individualized care plans
- Coordinating services to meet the unique needs of patients
Qualifications
To be successful in this role, you will need to meet the following requirements:
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
