We are seeking a dedicated Team Lead/Discharge Coordinator to enhance our services at Counseling & Recovery Services, LLC. This role necessitates a Bachelor's degree in social or human sciences from an accredited institution, along with a minimum of 5 years of professional experience in the mental health or related social service sectors.
Key Responsibilities:
- Conduct initial assessments and consultations with clients, ensuring appropriate referrals are made.
- Develop and implement comprehensive discharge plans, detailing the next steps in the client's treatment journey.
- Collaborate with clinical and administrative staff to facilitate seamless transitions for clients within the service delivery system.
- Maintain accurate records and update client data as required.
- Participate in regular team meetings to review cases and improve service delivery outcomes.
Qualifications:
- Certification as a Case Manager II is preferred or the ability to obtain it upon hire.
- A valid Oklahoma Driver's License and a clean driving record are essential.
- Strong knowledge of community resources and crisis management skills.
- Excellent communication skills, both verbal and written, with a commitment to maintaining confidentiality.
