Job Description

We're seeking a Patient Advocacy and Support Specialist to join our team, providing compassionate care and support to patients in their homes.

As a member of our interdisciplinary team, you'll work collaboratively with patients, families, and healthcare providers to develop individualized care plans and deliver high-quality patient care.

Responsibilities
  • Assess patient needs, identify areas for improvement, and develop targeted interventions to enhance patient outcomes.
  • Provide emotional support and education to patients and families, promoting empowerment and self-advocacy.
  • Collaborate with healthcare teams to ensure seamless care transitions, coordinate services, and manage patient records.
Requirements
  • A valid nursing license (RN or LPN) in the state of practice.
  • At least two years of nursing experience, preferably in home healthcare or community settings.
  • Strong interpersonal and communication skills, with ability to work effectively with diverse populations.
