As a Social Worker II with the Texas Department of Aging & Disability Services, you will play a vital role in ensuring the well-being and independence of individuals with intellectual and developmental disabilities.

Key responsibilities include:

  1. Providing complex social services, including liaison work between individuals, legally authorized representatives, and other stakeholders.
  2. Gathering information to assess support systems and service needs, participating in interdisciplinary team meetings as needed.
  3. Maintaining accurate social histories and updates.
  4. Supporting the assessment of individual rights and capacity to make decisions.
  5. Designing and providing individual and group training to promote the exercise of rights.
  6. Assisting with coordination of admissions, transfers, transitions, and discharges.

Requirements:

Ability to communicate effectively in writing and verbally, with strong knowledge of individuals with intellectual and developmental disabilities, human growth and development, and behavioral and mental health.

Ability to perform all job duties in accordance with the Texas State Board of Social Work Examiners Code of Conduct and National Association of Social Workers Code of Ethics.
