Position Overview:
RMD VillageHealth DM, LLC is seeking a dedicated Full-Time Registered Nurse (RN) Case Manager to join our team. This role is essential in supporting patients with complex healthcare needs, ensuring they receive comprehensive and coordinated care.
Key Responsibilities:
- Conduct thorough assessments to identify the medical, social, emotional, and financial needs of patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
- Develop and implement individualized care plans in collaboration with healthcare professionals and the patient’s support system.
- Facilitate effective communication among all members of the healthcare team to ensure continuity of care.
- Manage care transitions and address social determinants of health that may impact patient outcomes.
- Utilize telehealth technologies for remote patient monitoring and in-person visits as needed.
- Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
Qualifications:
- Current RN License is required; BSN is preferred.
- A minimum of 5 years of nursing experience is necessary.
- Certification in CPR or Basic Life Support (BLS) is mandatory.
- Proficient in computer skills, including MS Word, Excel, and Outlook.
- Ability to travel within an assigned territory for patient visits.
- Experience in case management or chronic care management is a plus.
- Spanish-speaking candidates are encouraged to apply.
