Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team at Riverside Health System. As a PRN RN Home Health Specialist, you will provide high-quality nursing care to patients in their homes, working under the supervision of our Nurse Manager and Director.

Key Responsibilities
  • Assess, plan, implement, and evaluate the total nursing care of assigned patients on an intermittent visiting basis.
  • Develop and implement individualized care plans, taking into account the patient's physical, psychosocial, and cultural needs.
  • Supervise and delegate tasks to non-professional staff, ensuring optimal patient care.
  • Collaborate with our multi-disciplinary team, including physical, speech, and occupational therapy, medical social services, and Home Health Aide services, to provide comprehensive care.
  • Complete all required clinical documentation, including OASIS assessments, patient notes, and verbal orders, in a timely and accurate manner.
Requirements
  • Program Graduate, Registered Nurse (Required)
  • 2 years Registered Nurse experience (Required)
  • Registered Nurse (RN) licensure in the state of practice (Required)
  • CPR/BLS certification (Required)
