Job Responsibilities

Our Home Health Risk Assessment Nurse Practitioners conduct in-depth assessments of adult and geriatric patients, including medical history, diagnosis, treatment, health education, and case management referrals.


You will work collaboratively with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care.


Additionally, you will provide services in a variety of settings, including home visits, skilled facility visits, and telehealth visits.

As a Home Health Risk Assessment Nurse Practitioner, you can expect a competitive compensation package, with pay rates of $90.00 or $95.00 per completed assessment, based on county.


We also offer flexible scheduling, allowing you to choose when and how much you want to work.

Requirements

To be successful in this role, you will need to have a Master's degree in nursing or a commensurate level of experience and satisfactory completion of NP licensure.


You must also be a licensed RN and NP in the state of practice, with prescription authority or the ability to obtain it.


Board certification in a Matrix-approved specialty is also required, as is current BLS, ACLS, or CPR certification.


Additionally, you will need to have a minimum of 1 year of experience as a Nurse Practitioner and strong computer skills, including familiarity with employee health and medical record software.


You will also need to possess excellent verbal and written communication skills and be comfortable with frequent change.


Finally, you will need to have a valid state driver's license and be able to drive a car, with proof of adequate automobile insurance coverage for the state of residence.
