Job Title: Nurse Practitioner, Home Visits

Job Summary:

Optum is seeking a skilled Nurse Practitioner to join our Home-Based Medical Care team in Columbiana/Salem, OH. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and develop personalized care plans to address their unique needs and goals.
  • Collaborate with patients, caregivers, and healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Prescribe appropriate diagnostics and interventions to prevent unnecessary hospitalizations and promote effective disease management.
  • Provide education and support to patients and their families to empower them to manage their conditions effectively.
  • Participate in ongoing quality improvement initiatives to ensure the highest standards of care and promote clinical excellence.

Requirements:

  • Completion of a Nurse Practitioner or Physician Assistant accredited program.
  • Current NP/PA license and access to reliable transportation.
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
  • Knowledge of CPT, ICD-10, and HCPC codes.
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
