**About the Role:**
We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services for seniors and individuals with complex needs.
**Responsibilities:
- Provide evidence-based medical care to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.
- Collaborate with caregivers and other healthcare professionals to develop comprehensive care plans.
- Order and interpret laboratory and diagnostic testing as needed.
- Ensure patient medical records are up-to-date.
**Requirements:**
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
