As a Geriatric Care Specialist at Abode Care Partners, you will play a vital role in delivering high-quality medical care to seniors and patients with complex needs.
About the RoleThis position offers a unique opportunity to utilize your clinical expertise and compassion to provide evidence-based care. You will work collaboratively with other healthcare professionals to manage chronic and urgent illnesses, develop comprehensive care plans, and ensure seamless patient transitions.
Key Responsibilities- Provide evidence-based medical care to seniors and patients with complex needs
- Develop and implement comprehensive care plans to address chronic and urgent health issues
- Collaborate with interdisciplinary teams to ensure coordinated care and effective communication
- Maintain accurate and up-to-date patient medical records
To be successful in this role, you will need:
- A Master's degree in Nursing (MSN)
- An active state Nurse Practitioner license
- Current National Board Certification as a Nurse Practitioner
- Active state DEA licensure
- A minimum of five years of nursing experience, with at least two years as a nurse practitioner
- Excellent interpersonal and communication skills, both verbal and written
- Strong time management and organizational skills, with ability to prioritize tasks effectively
