This position offers a unique opportunity to utilize your clinical expertise and compassion to provide evidence-based care. You will work collaboratively with other healthcare professionals to manage chronic and urgent illnesses, develop comprehensive care plans, and ensure seamless patient transitions.

Key Responsibilities
  • Provide evidence-based medical care to seniors and patients with complex needs
  • Develop and implement comprehensive care plans to address chronic and urgent health issues
  • Collaborate with interdisciplinary teams to ensure coordinated care and effective communication
  • Maintain accurate and up-to-date patient medical records
Requirements

To be successful in this role, you will need:

  • A Master's degree in Nursing (MSN)
  • An active state Nurse Practitioner license
  • Current National Board Certification as a Nurse Practitioner
  • Active state DEA licensure
  • A minimum of five years of nursing experience, with at least two years as a nurse practitioner
  • Excellent interpersonal and communication skills, both verbal and written
  • Strong time management and organizational skills, with ability to prioritize tasks effectively
