About the Position:

The Salvation Army USA Central Territory is seeking a skilled Pathway of Hope Case Manager to provide strength-based case management services to individuals and families. This is a regular full-time, non-exempt position that requires a minimum of two years of progressive experience in case management and client support in the social services profession or related educational experiences.

About You:

We are looking for an individual with excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member. A bachelor's degree in social work, sociology, criminal justice, or other related field is preferred.

Your Responsibilities:

  • Conduct outreach and engagement services in the community to identify and support participants
  • Engage and build rapport with the target population and meet desired outcomes of eligibility requirements
  • Collaborate with local leadership to best support clients and their designated goals
  • Provide appropriate referrals and linkage to support individuals and families in the local community
  • Maintain written and electronic case files with all relevant client information, advocacy, and financial services as necessary

Requirements:
