As a RN Case Manager, Home Health Professional with the Good Samaritan Society, you will play a vital role in integrating healthcare services for clients from pre-admission to post-discharge. This involves coordinating and sequencing client care to enhance patient/resident flow and promote interdisciplinary communication.
Key Responsibilities- Utilize independent nursing judgment to integrate healthcare services
- Develop and implement structured care methodologies to meet the unique needs of clients
- Collaborate with healthcare teams to coordinate care and promote early intervention
- Monitor and evaluate patient/resident responses to care, identifying areas for improvement
- Fiscally responsible, controlling variances and duplication in care
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting
- Currently holds an unencumbered RN license with the State Board of Nursing
- Obtains and maintains required department-specific competencies and certifications
