We are seeking a skilled RN - Mechanical Heart Coordinator to join our team at Providence Sacred Heart Medical Center & Children's Hospital. As a key member of our cardiovascular team, you will play a vital role in coordinating transplant-related activities for patients across the continuum of care.
Key Responsibilities- Coordinate case management for transplant patients, ensuring seamless care from pre-transplant to post-transplant.
- Provide patient and family education, ensuring they are well-informed and prepared for transplant procedures.
- Maintain waiting list registration and ensure patients are ready for deceased donor transplants.
- Arrange transplant admissions, monitor clinical pathway progression, and provide post-discharge follow-up.
- Participate in the development of patient education programs and provide professional/peer education.
- Maintain 24-hour on-call coverage for organ offers and other transplant needs.
- Prepare for and participate in outreach clinics with physicians.
- Participate in recipient selection for placement on the UNOS wait list as part of the transplant committee.
- Communicate with patients, nephrologists, and dialysis programs regarding transplant decisions.
- Provide inpatient and outpatient clinic transplant education.
- Monitor labs, arrange outpatient infusions and radiologic procedures, and address patient needs in the post-transplant patient clinic.
- Communicate and set up initial post-transplant visits with primary transplant physicians.
- Assist with program development and awareness, research, and other duties as delegated.
- Bachelor's Degree in Nursing.
- Washington Registered Nurse License.
- Remains up to date on UNOS-OPTN and CMS regulations.
- 5 years of Acute care nursing experience.
- Certified Clinic Transplant Coordinator (CCTC) certification.
- 3 years of Current experience in Cardiology nursing, including transplantation.
