Job Summary

The Senior Wellness Navigator is responsible for providing case management services to older adults who are at increased risk of losing their independent status due to a poorly controlled respiratory condition or increased fall risk. The Senior Wellness Navigator will utilize the case management model based on guidance by the Centers for Disease Control and Prevention.

Key Responsibilities
  • Implement and manage health education strategies, interventions, and programs for clients.
  • Develop, monitor, and assess health communication interventions and policies.
  • Assess clients' needs and make referrals, as necessary.
  • Communicate and advocate effectively for health and health education with clients and within the community.
  • Develop partnerships and work collaboratively with organizations that serve the client base.
  • Develop strong educational materials to educate the public and partners regarding services.
  • Attend meetings and provide education to community organization meetings, neighborhood meetings, and government agencies.
  • Coordinate activities with other governmental agencies and with community organizations.
  • Protect privacy information in accordance with HIPAA regulations.
  • Assess client and organization needs for health education, including change readiness.
  • Participate in public health emergency preparedness exercises and in response to public health emergencies as directed.
  • Translate scientific language and concepts into clear, simple, and understandable information for clients and community organizations to maximize the potential for them to take reasonable action.
  • Help develop, identify, implement, and improve client education material and other tools that encourage healthy decisions.
  • Comply with training and practice requirements for Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Maintain knowledge of Marion County Public Health Department services and programs and communicate that information to members of the diverse community.
  • Participate in public health emergency preparedness exercises and in the response to public health emergencies, as directed.
  • Participate in a minimum of two Health Outreach Promotion and Education (H.O.P.E.) events totaling a minimum of eight (8) hours annually.
Qualifications

Bachelor's Degree Required. Concentration in Public Health, Social Work, or closely related area preferred.

Licenses/Certifications Required

When eligible, maintain and Healthy Homes Specialist credential. Valid Indiana Driver's License.

Knowledge, Skills & Abilities

Strong written and verbal communication and interpersonal skills. Detail-oriented and self-motivated. Able to listen well, accurately communicate ideas, and to work well in a team.

Working Environment

This is a driving position and requires a significant percent of work time to be performed outside the base office. Professional settings including health care entities throughout the county. Office time includes confined sitting, prolonged computer screen time, and steady use of hands and fingers. Annual health vaccinations are required as a condition of employment. Certain exemptions may apply with prior approval and documentation.
