About the Role

The Clinical Care Navigator will play a pivotal role in coordinating patient care, ensuring seamless transitions, and promoting effective disease management. This dynamic professional will work closely with patients, families, healthcare providers, and community partners to develop and implement comprehensive care plans.

Key Responsibilities
  • Promote health equity by identifying and addressing social determinants of health that impact patient outcomes.
  • Conduct thorough assessments of patient needs, including clinical, social, functional, and continuing care requirements.
  • Develop and implement personalized care plans, incorporating short-term and long-term goals, to optimize patient outcomes.
  • Foster collaborative relationships with healthcare providers, patients, and families to ensure timely communication and effective coordination of care.
  • Cultivate partnerships with community resources to enhance access to necessary services and support.
  • Maintain accurate and up-to-date records of patient interactions, progress, and outcomes.
Requirements
  • At least two years of experience in nursing or a related field.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse populations.
  • Strong problem-solving and critical thinking abilities, with a focus on patient-centered care.
  • Proficiency in electronic medical records and other relevant software systems.
  • Bachelor's degree in Nursing or a related field; Master's degree preferred.
Compensation and Benefits

The salary for this position is estimated to be around $65,000-$85,000 per year, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and opportunities for professional growth and development.
