Home Health Registered Nurse RN Full Time

Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse RN to join our team. As a Home Health Registered Nurse RN, you will provide high-quality nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans.
Responsibilities:
- Develop and implement individualized care plans for patients, including medical interventions and measurable goals and outcomes.
- Provide education and support to patients and their families, empowering them to manage their care and make informed decisions.
- Collaborate with primary caregivers to ensure optimal functioning levels for each patient.
- Coordinate the continuum of care through timely communication with physicians and other caregivers.
- Provide skilled nursing care in patients' homes, utilizing a range of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
Requirements:
- An active RN license in the state of application.
- Valid CPR certification.
Preferred Qualifications:
- Medicare Skilled Nursing experience.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
