Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse RN to join our team. As a Home Health Registered Nurse RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities:
- Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare teams.
- Provide skilled nursing care, including wound care, infusion therapy, and catheter management.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress, identify areas for improvement, and adjust care plans accordingly.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
- 1 year of experience in a healthcare setting, preferably in home health or skilled nursing.
Preferred Qualifications:
- Medicare skilled nursing experience.
- Basic understanding of Oasis.
