Job Summary

The Mental Health Social Practitioner will be responsible for providing therapeutic support and advocacy to members, helping them to achieve their goals and improve their overall well-being. This will involve working closely with members to identify their strengths and challenges, and developing personalized plans to support their recovery.

Key Responsibilities
  • Provide therapeutic support and advocacy to members, helping them to achieve their goals and improve their overall well-being.
  • Work closely with members to identify their strengths and challenges, and develop personalized plans to support their recovery.
  • Support members in their transitional employment opportunities, and provide training and support to help them develop new skills.
  • Assist members with entitlements and benefits, such as food stamps, SSI/SSDI, and Medicaid/Medicare.
  • Facilitate supported housing applications and work to find appropriate housing for members.
  • Advocate for members' well-being, safety, and livelihood, and provide support around clinical, legal, housing, appointments, medications, and independent living opportunities.
Requirements
  • Undergraduate degree or an equivalent combination of training and experience required.
  • Graduate degree or equivalent preferred.
  • 1-3+ years' experience working with individuals with serious mental illness and/or hands-on experience working in a community setting preferred.
  • 1-2 years coaching and counseling experience preferred.
  • 1-2 years of vocational rehabilitation, training, or employment specialist experience preferred.
Working Conditions

This is a full-time position, Monday - Friday, 9AM-5PM, with some flexibility as needed. The salary is $29.12 per hour.
