About the Role

We are seeking a skilled Registered Nurse to join our team at Commonwealth Community Based Services in Lexington, KY. As a Registered Nurse, you will provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.

Key Responsibilities
  • Provide clinical nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
  • Ensure patient eligibility and medical necessity for services, and initiate plan of care revisions as needed with physician approval.
  • Collaborate with other community providers to develop and revise individualized plans of care and service plans.
Requirements
  • Current RN licensure in the state of practice.
  • At least one year of clinical experience as a Registered Nurse, or recent clinical experience as an LPN with a waiver.
  • Current CPR certification.
