Position Overview

The role involves accurately preparing and labeling medications, offering drug-related guidance to patients, hospital staff, and medical professionals, and overseeing the operations of pharmacy support personnel.

Key Responsibilities

Memorial Hermann's Outpatient Cancer Center is in search of a staff pharmacist to enhance our operations.

This position will focus on outpatient chemotherapy services.

Experience in a hospital or chemotherapy setting is preferred, though training will be provided.

Work Schedule:

0930-2000 (10-hour shifts, 4 days per week, rotating weekends)

or

1030-1900 (8-hour shifts, 5 days per week, rotating weekends)

This is a full-time position eligible for a comprehensive benefits package, including medical and dental coverage, tuition reimbursement, and a 401K plan.

Minimum Qualifications

Education: Graduate of an accredited School of Pharmacy.

Licenses/Certifications: Must be licensed as a Registered Pharmacist in the State of Texas; Pharmacy Sterile Product certification is required if the role involves compounding sterile products.

Experience / Knowledge / Skills:

  • A minimum of two (2) years of experience as a hospital pharmacist, or as determined by the System Executive, with a solid understanding of unit dose medication and I.V. admixture processes.
Core Responsibilities
  • Accurately fills and labels prescriptions while providing drug-related information and education to in-patients, out-patients, hospital staff, and medical professionals.
  • Dispenses controlled substances and maintains precise inventory records.
  • Supervises pharmacy technical staff and mentors pharmacists, pharmacy students, or residents.
  • Engages in the Medication Use Evaluation Program and the Process Improvement Program.
  • Maintains medication profiles, reviews them for drug-related issues, and offers consultation on drug research.
  • Oversees the scheduling and preparation of I.V. solutions and additives.
  • Reports medication errors or adverse drug interactions.
  • Takes full responsibility for department management in the absence of the Pharmacy Manager or Lead Pharmacist.
  • Collaborates with other professionals to create newsletters and drafts policies or procedures for review by the Pharmacy Director.
  • Conducts clinical interventions, which may include medication profile reviews, IV to oral conversions, renal dosing adjustments, antimicrobial streamlining, patient counseling, obtaining medication histories, vancomycin/aminoglycoside dosing, therapeutic drug monitoring, and participation in multidisciplinary rounds.
  • Documents clinical interventions thoroughly.
  • Ensures safe care for patients, staff, and visitors; adheres to all Memorial Hermann policies, procedures, and standards while managing budgetary constraints, including time management, supply management, productivity, and quality of service.
  • Promotes individual professional growth and development by fulfilling requirements for mandatory and continuing education and skills competency; supports department-based goals that contribute to the organization's success; serves as a preceptor, mentor, and resource for less experienced staff.
