We are seeking a compassionate and dedicated RN to join our hospice team as an End-of-Life Care Specialist. In this role, you will provide high-quality care to patients in their homes or assisted living settings.

About the Role:

  • Develop and implement individualized care plans to address patient needs
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Supervise LPNs and Home Health Aides to ensure quality care delivery

Requirements:

  • Current RN License
  • Hospice and/or home care experience preferred
