Job Summary:
We are seeking an experienced Engagement Nurse Practitioner to join our team at WellBe Senior Medical. As a key member of our interdisciplinary care team, you will be responsible for delivering high-quality, patient-centered care to our geriatric patients in their homes.
Responsibilities:
- Deliver primary care in the home-based setting for patients with chronic and acute illnesses and injuries.
- Complete initial visits and assessments to onboard eligible patients onto our care model.
- Ensure comprehensive medical records are maintained through accurate and timely documentation.
- Partner with other clinicians to manage patient health and wellbeing.
- Facilitate staff, patient, and family decision-making through educational resources.
- Collaborate with community teams to complete welcome visits and care assessments.
- Deliver ongoing care information, counseling, and guidance to patients with critical conditions.
- Refer patients to internal and external resources as needed.
Requirements:
- Previous experience in home care, primary care, hospice, palliative care, or geriatrics.
- Valid driver's license, car insurance, and access to a car.
- Proficiency with Microsoft Office, Electronic Health Records, and Scheduling Software.
- Experience in diagnosing and treating ongoing, chronic, and urgent medical/psychological conditions of older adults.
Travel Requirements:
Travel may be required up to 100% locally to deliver patient care.
