Job Summary:
We are seeking a skilled Chronic Care Nurse to join our team at ICONMA's Healthcare and Clinical division. As a Chronic Care Nurse, you will be responsible for providing high-quality patient care in a dialysis facility setting. Our team is comprised of highly specialized recruitment professionals who are passionate about the healthcare and clinical industry.
Responsibilities:
* Provide patient care in a dialysis facility setting
* Collaborate with healthcare professionals to develop and implement patient care plans
* Conduct patient assessments and develop individualized care plans
* Educate patients and families on disease management and treatment options
Requirements:
* RN license
* BLS certification
* 1 year of Nephrology Nursing experience in the last two years
