**Salary Range:** $77,000 - $85,000 annually, depending on experience and qualifications.
**Job Description:**
We're seeking an experienced Registered Nurse (RN) to join our team as a Patient Care Navigator. As a key member of our interdisciplinary care team, you will be responsible for coordinating patient care, communicating with healthcare professionals, and ensuring seamless transitions between care settings.
**Responsibilities:**
- Perform routine home visits to assess patient needs and develop personalized care plans;
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure timely interventions;
- Maintain accurate and up-to-date patient records, including medication profiles and lab results;
- Participate in weekly case conferences and monthly case manager meetings to review patient progress and adjust care plans as needed;
- Develop and maintain relationships with patients, families, and caregivers to ensure continuity of care;
- Stay current with industry developments and best practices in patient care navigation.
**Qualifications:**
To be considered for this role, you must have:
- A valid state license as a Registered Nurse (RN);
- At least one year of clinical experience in a home health or hospice setting;
- Demonstrated knowledge and skill in current nursing practice;
- A valid state driver's license and automobile liability insurance.
