Key Responsibilities:
  • Conduct in-home assessments to identify and address health risks
  • Develop and implement personalized care plans to improve patient outcomes
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Stay up-to-date with the latest clinical guidelines and best practices
Requirements:
  • Active NP or PA license or ability to obtain by start date
  • Active national NP or PA certification or ability to obtain national certification in state of assignment by start date
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs
  • Reliable transportation to complete home visit assessments
Preferred Qualifications:
  • 1+ years of clinical experience in a home health care setting
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home Health care or home visit experience

We offer a competitive compensation package, including a $40,000 Student Loan Repayment or $30,000 Sign-on Bonus for individuals who have not previously participated in this program. Additionally, you'll have the opportunity to work in a flexible schedule, with no on-call, weekends, or holidays required.
