Clinical Care Coordinator

The estimated salary for this position is $75,000 - $95,000 per year, depending on experience. To succeed in this role, you will need to have a strong foundation in nursing principles, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
• Conduct initial and ongoing comprehensive patient assessments
• Develop and implement individualized care plans in collaboration with the patient, family, physician, and interdisciplinary team
• Perform skilled nursing tasks such as wound care, Foley catheter care, IV therapy, and medication administration
• Monitor and document patient progress and response to treatments and interventions
Requirements:
• Education: Graduate of an accredited nursing program (Associate's Degree or Bachelor's Degree in Nursing)
• Licensure: Current RN license in the state of practice (Massachusetts)
• Experience: Minimum of 1 year of clinical nursing experience, preferably in home health or a related field
• Skills: Strong assessment, communication, and organizational skills
