We are seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team at Optum, a leading health care company. As a HouseCalls Clinical Provider, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities- Conduct comprehensive in-home assessments, including medical history reviews, physical exams, and vital sign checks.
- Perform evidence-based practice screenings, such as point-of-care testing, to identify potential health risks.
- Develop and implement personalized care plans to address patients' medical needs and promote healthy outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Identify and address urgent and emergent situations, providing timely interventions as needed.
- Educate patients on disease management, medication adherence, and healthy lifestyle choices.
- Active NP or PA licensure, with the ability to obtain national certification in the state of assignment.
- 1+ years of clinical experience in a relevant setting, with a strong foundation in gerontology, cardiology, internal medicine, or endocrinology.
- Reliable transportation and the ability to navigate various dwelling conditions.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages is a plus.
