About the Role:

As a Registered Nurse in Home Health, you will provide high-quality, patient-centered care to individuals in their homes. Your primary focus will be on delivering comprehensive nursing evaluations, developing personalized care plans, and coordinating with other healthcare professionals to ensure seamless care transitions.

Responsibilities:
  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Coordinate with physicians, agency personnel, and community resources to ensure continuity of care
  • Provide education and support to patients and families on disease management, medication adherence, and self-care
  • Document patient progress and outcomes accurately and timely
Requirements:
  • Current RN licensure in the state of Oregon
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
