Licensed Practical Nurse

This position is responsible for providing high-quality patient care to individuals in their homes. As a Licensed Practical Nurse (LPN) with Critical Nurse Staffing, you will be part of a dynamic team dedicated to delivering exceptional care and improving patient outcomes.
Key Responsibilities
Provide direct patient care as described in state regulations for LPN/LVN, in all cases involving nursing services.
Assist in the implementation of changes in the Plan of Care for the improvement of client care.
Make the initial evaluation visit as applicable and re-evaluate the patient's needs on a regular basis.
Provide nursing services, treatments, and diagnostic procedures requiring specialized skills and assuming full responsibility for each assigned patient.
Participate in in-service programs, as well as training, and teaching other nursing personnel as required.
Assist client in learning appropriate self-care techniques.
Promote and maintain client health through teaching and appropriate rehabilitative measures.
Develop and implement the treatment plan for each patient under the direction of their physician and plan of care.
Observe and report symptoms, reactions to treatments, drugs, and changes in the patient's physical and/or emotional condition.
Initiate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety.
Maintain clinical and progress notes for each assigned patient receiving care and providing progress reports to their physician.
Coordinate services for patients, as needed, to other agencies and health care providers as appropriate.
Counsel the patient and their family in meeting nursing and related needs,< Provide supervision of Home Health Aide/Home Care Aide/Certified Nursing Assistant services and prepares written instructions for care provided by HHA/HCA/CNA.
Demonstrate self-direction and ability to work without the presence of immediate supervisor.
Consider age-related factors with patient care for the adult and elderly when providing home care.
Maintain accurate time records.
Submit required documentation in a timely manner.
Report observed or suspected child or adult abuse pursuant to mandated requirements.
Requirements
Must be able to read, analyze, and interpret general business practices, policies and procedures, professional journals, technical guidelines, and governmental regulations.
Must be able to write reports, business correspondence, and procedure manuals.
Must be able to effectively present information and respond to questions from groups of managers, clients, customers, and the public.
Must be able to provide guidance and direction accurately and clearly to Company staff and employees.
Education/Training
Licensed Practical Nurse/Licensed Vocational Nurse license to practice professional nursing in each state where patient care is supervised, current Basic Cardiac Life Support certification.
Valid Driver's License
Valid and current CPR Certification
Valid and current License Practical Nurse license in the state of practice
