About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a key member of our interdisciplinary team, you will play a vital role in providing patient-centered care and support to our patients and their families.
Key Responsibilities:
- Provide high-quality patient care and support to patients in their homes, including assessing patient needs, developing and implementing care plans, and coordinating with other healthcare professionals as needed.
- Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and other health-related topics.
- Conduct regular home visits to assess patient progress, provide ongoing support, and make adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and transitions.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
- RN license in the state of employment
- Two years of experience as a registered nurse, with a preference for home health experience
- Current driver's license
- Current CPR and BCLS certification
