Key Responsibilities- Assist in assessment and care planning for residents, ensuring their physical and emotional needs are met.
- Coordinate and supervise all services on assigned units, maintaining a safe and supportive environment.
- Regularly round on nursing units to ensure residents receive timely and effective care.
- Facilitate communication between licensed nurses and providers, promoting seamless care coordination.
- Ensure complete and accurate documentation through spot-checking and ongoing supervision.
- Consult with Unit Managers to provide continuity of care to residents.
- Assist in direct resident care and work as a staff nurse, providing individualized attention and support.
- Current MA Licensed Practical Nurse (LPN) licensure.
- Current BLS certification.
- At least 1 year of Long-Term Care experience.
