Licensed Practical Nurse

This role focuses on providing high-quality patient care services in accordance with physician orders. As a Licensed Practical Nurse, you will be part of the healthcare team, working under the supervision of a registered nurse to deliver safe and effective therapy to assigned patients.
Key Responsibilities- Implement and evaluate patient care plans, ensuring the highest level of care for patients.
- Participate in the implementation and evaluation of patient care, working collaboratively with the healthcare team.
- Provide direct patient care, maintaining patient rights and dignity at all times.
- Perform technical aspects of patient care, including medication administration and procedures, safely and according to established policies and procedures.
- Assess and reassess patient pain, utilizing appropriate pain management techniques and documenting pain management.
- Communicate changes in patient condition to the registered nurse immediately.
- Deliver care in a fiscally responsible manner, establishing a positive relationship with patients and families.
- Provide teaching and counseling to patients and families, documenting teaching on the appropriate teaching tool.
- Review and educate discharged patients and families on ADLs, medications, treatments, and provide discharge instruction sheets.
- Document all nursing interventions and patient responses in the medical record.
- Delegate responsibilities to others, understanding their roles, knowledge, and capabilities.
- Participate in performance improvement activities, utilizing performance improvement principles to support and improve patient care.
- Maintain a collaborative work relationship with peers and colleagues, creating a positive work environment.
- Massachusetts's Licensed Practical Nurse (LPN) license.
- Experience dependent upon unit requirements and needs.
- Basic Life Support Certification.
- Cardiopulmonary Resuscitation (CPR) Instructor.
- Graduation from an accredited practical nursing program.
