Primary Care Physician

Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a Family, Internal, or Geriatric Medicine Physician to join our team in Palm Coast, Florida.
About the RoleWe are looking for a dedicated and compassionate physician to provide high-quality patient care in our outpatient office. As a Primary Care Physician, you will have the opportunity to take a hands-on approach, addressing patient needs inside the exam room and looking beyond to understand how social determinants of health are impacting health outcomes.
Responsibilities- Provide comprehensive primary care to patients, including routine check-ups, health screenings, and disease management.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with a multidisciplinary team, including Advanced Practitioners and a Care Team, to provide coordinated care.
- Participate in daily huddles to discuss patient cases and share best practices.
- Act as a lead and active participant in the Care Team, providing medical expertise and guidance.
- Doctor of Medicine or Osteopathic Medicine degree.
- Board Certified/Board Eligible in Family, Internal, or Geriatric Medicine.
- Valid, unrestricted license in the state of desired employment (or willing to obtain).
- Current, unrestricted DEA license.
- Two to five years directly applicable experience preferred.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.
- Guaranteed base salary + 20% bi-annual quality incentive, total compensation up to $300,000.
- Excellent benefit package, including health insurance, CME allowance/time, occurrence-based malpractice insurance, relocation and sign-on bonus options, 401(k) with employer match, life insurance/disability, paid time off/holidays, and minimal call.
We offer a dynamic and supportive work environment, with a focus on patient-centered care and team collaboration. Our office is located in Palm Coast, Florida, and we offer a flexible schedule with Monday-Friday 8:00am-5:00pm hours, with no weekends or holidays.
